The VisitPlanner is a tool designed tohelpparents of children with Inflammatory Bowel Disease managetheirchild’s health and optimize their engagement in the healthcareprocess. It encourages parents to observe their child’shealthcondition and to keep track of important data to discuss withtheirchild’s care providers. The tool is designed to be usedroutinelythroughout the time period between clinic visits. Itincorporatesongoing assessment in between visits with focusedpreparation inthe week prior to a scheduled clinic visit. The toolusesnotifications to prompt parents to complete brief assessmentsoftheir child’s health status once per week, on random days.Parentsalso receive weekly notifications regarding anynon-urgentquestions or concerns they would like to remember toaddress attheir child’s next clinic visit. One week prior to ascheduledclinic visit, parents are able to view a summary of thedata theyhave recorded and complete a pre-visit plan that isreturned to theapp in report form to be used at their visit andalso deliveredelectronically to their healthcare team.
